All fees are non-refundable. Your daughter must have a current physical on file and her fees be paid in full before being allowed to participate in the 2024-2025 season. Failure to pay by the due date of January 30, 2025 may result in your daughter not being allowed to take the field. You, the parent, guardian, and player also agree to the following terms:
1. I agree and release Ooltewah Lady Owls Lacrosse Club, coaches, and board members of any liability for injury, illness, or loss arising out of my daughter's participation in the club including but not limited to conditioning, practice, play and travel to or from practice or games.
2. I recognize that there are inherent risks associated with the conditioning for practice of and play of lacrosse. I understand that lacrosse is a physically demanding contact sport. I hereby represent to Ooltewah Lady Owls Lacrosse that my daughter is in good physical condition and has no known medical limitations that prevent her from engaging in such activities.
3. In the event of any injury of illness, I authorize Ooltewah Lady Owls Lacrosse Club to seek medical treatment and I will be financially responsible for any and all charges so incurred, including but not limited to emergency transportation, physician/medical care giver, and hospital charges.
